
The Integrity of Tissue Healing
When it comes to getting rid of pain it’s all about integrity. What? That’s right; I said “it’s all about integrity!” And it doesn’t matter if your foot or ankle is sore or it’s your shoulder or thumb. Because when it comes to healing inflamed tissues, the integrity, or strength and health of those tissues must be preserved. In fact, that is the biggest difference between what therapy does and surgery and injections do.

Let me explain. Let’s say you have Achilles pain, you know where your calf attaches to the back of your heel, plantar fascitis, rotator cuff tendonitis, or thumb pain, whatever. It is usually a case of the tissue being compromised in some fashion. It may be due to overuse, bad posture, muscle weakness or imbalance. The result initially is inflammation, damaged tissue and the nerves in and around this area are irritated. In this case the tissue promotes growth, lots of it in fact. The area becomes swollen with fluid to heal the area. It if goes on long enough the tissue can begin to degenerate. It goes from red, inflamed and swollen to a dusky grey; the tendon begins to fray and breakdown. This is called tendonosis, not tendonitis.
In therapy at TriState Hand, Foot and Ankle Therapy we try to determine how this happened. Bad posture or a muscle imbalance; which is pulling the bones too much one way that causes rubbing and inflammation. I call this “bad body geometry”. Maybe in the case of a painful Achilles (heel cord), you pronate (flat footed) too much, or too little! In any case it takes stretching what is too tight, and strengthening the muscles that are weak; or correcting the body mechanics (geometry) with orthotics or proper footwear. Then the tissues can regain their integrity and heal properly. So you see it is all about the integrity when it comes to tissue healing in therapy. Please understand, what has taken a long time to develop will take time and exercise to heal. To re-grow healthy protein in the tendons, muscles and joints takes time, usually six weeks. People often want it to get better fast but the truth is it takes time to get tissue that has integrity and health back.
